HBSM Medical Release Form:
The Medical Release & Permission Form is needed for all events you attend and/or participate in with HBSM, and it is good for one year (August 1 to July 31). Once you have completed the form, please give it to Maggie Driggers to be uploaded to your Personal Youth Tracker profile:

REIMBURSE and/or REQUEST FOR FUNDS:
For church related events, funds may be requested from student accounts to cover expenses upcoming or for reimbursement of expenses already accrued. Download the proper form(s), fill out the RED SECTIONS, and return it to Mrs. Leah Hancock. You may email these completed forms to her. Requests must be submitted at least one week prior to the date they are needed.
Median Release:
Before HBSM can place a picture of any person online, the right to do so must be approved by that person, and if under 18, by their legal guardians. Please download the Media Release form below, sign it, and return it via email to Maggie Driggers. Her email address is maggie@horsebranch.org. Once we have received your signed form, it will be uploaded onto your personal Youth Tracker profile, and can be viewed at the bottom of the page under documents. If you should ever want to stop having your pictures posted, complete another form instructing us not to post your pictures. Thanks, Bro Brian
